{"id":7565,"date":"2025-04-29T11:43:11","date_gmt":"2025-04-28T23:43:11","guid":{"rendered":"https:\/\/websitedesignhosting.co.nz\/motovated\/?p=7565"},"modified":"2025-04-29T11:43:11","modified_gmt":"2025-04-28T23:43:11","slug":"hazard-identification-is-about-more-than-just-normal-operations","status":"publish","type":"post","link":"https:\/\/websitedesignhosting.co.nz\/motovated\/hazard-identification-is-about-more-than-just-normal-operations\/","title":{"rendered":"Hazard Identification Is About More Than Just Normal Operations"},"content":{"rendered":"<div class=\"wpb-content-wrapper\"><p>[vc_row][vc_column][vc_column_text css=&#8221;&#8221;]<span data-css=\"tve-u-17a35ffbd88\">In the years since I started doing work in the engineering <\/span><a class=\"tve-froala fr-basic\" href=\"https:\/\/www.motovated.co.nz\/health-and-safety-engineering\/\" target=\"_blank\" rel=\"noopener\" data-css=\"tve-u-17a360004e0\"><span data-css=\"tve-u-17a35ffbd90\">health and safety<\/span><\/a><span data-css=\"tve-u-17a35ffbd93\">\u00a0field, I\u2019ve worked with a huge number of organisations, either to identify hazards and manage risk proactively or to investigate and determine root causes after an incident has occurred.\u00a0 In cases where we were helping identify the root cause of an incident, one cause has come up over and over again.<\/span>[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/2&#8243;][vc_column_text css=&#8221;&#8221;]Too often, even if a hazard ID process is carried out, this fails to identify the full range of hazards \u2013 usually because entire failure modes or operational phases have simply not been considered.\u00a0 This results in a list of identified hazards that are then managed, but there isn\u2019t much value in that management if there are hazards that haven\u2019t even been identified.<\/p>\n<p>For example, I\u2019ve handled a lot of investigations that ultimately boil down to questions of\u00a0<a class=\"tve-froala fr-basic\" href=\"https:\/\/www.motovated.co.nz\/mechanical-engineering-certification-and-compliance\/\" target=\"_blank\" rel=\"noopener\" data-css=\"tve-u-17a35ffe99f\"><span data-css=\"tve-u-17a35ffbd95\">machinery safety<\/span><\/a><span data-css=\"tve-u-17a35ffbd97\">. \u00a0In those cases, I\u2019ve commonly found that a fairly comprehensive job was done of identifying hazards during normal operation of the machine, but hazard ID is usually less robust for the other operational phases \u2013 maintenance, setup, cleaning, etc. \u00a0In other machinery cases, foreseeable misuse often isn\u2019t considered \u2013 hazard ID is carried out on the assumption that people will be strictly adhering to\u00a0<\/span><a class=\"tve-froala fr-basic\" href=\"https:\/\/en.wikipedia.org\/wiki\/Standard_operating_procedure\" target=\"_blank\" rel=\"noopener\" data-css=\"tve-u-17a35fff5c6\"><span data-css=\"tve-u-17a35ffbd9c\">SOP<\/span><\/a><span data-css=\"tve-u-17a35ffbd9e\">s, which is generally not a sound assumption.<\/span><\/p>\n<ul>\n<li style=\"list-style-type: none;\"><\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/2&#8243;]<div class=\"ult-spacer spacer-6a633c0c808b8\" data-id=\"6a633c0c808b8\" data-height=\"0\" data-height-mobile=\"0\" data-height-tab=\"0\" data-height-tab-portrait=\"\" data-height-mobile-landscape=\"\" style=\"clear:both;display:block;\"><\/div>[vc_single_image image=&#8221;7373&#8243; img_size=&#8221;full&#8221; css=&#8221;&#8221;][\/vc_column][\/vc_row][vc_row][vc_column][vc_column_text css=&#8221;&#8221;]Other examples include the use of mechanical equipment to lift suspended loads \u2013 I\u2019ve dealt with a few cases over the years where equipment had been selected based on required strength to support the loads, but hazards related to stability simply hadn\u2019t been considered.\u00a0 In one case, this failure to consider stability as well as strength led to a fatality.[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/2&#8243;][vc_column_text css=&#8221;&#8221;]As a field,<a class=\"tve-froala fr-basic\" href=\"https:\/\/www.motovated.co.nz\/mechanical-engineering-certification-and-compliance\/\" target=\"_blank\" rel=\"noopener\" data-css=\"tve-u-17a35ffd8d9\"><span data-css=\"tve-u-17a35ffbda1\">\u00a0health and safety<\/span><\/a><span data-css=\"tve-u-17a35ffbda3\">\u00a0has a lot of best practice and guidance on how to assess risk and what can be done to control risk, but all of that is basically useless if hazards aren\u2019t identified in the first place.\u00a0 Hazard ID can sometimes seem like the boring first step before you get to the fun part of designing new controls, but since hazard ID is foundational it\u2019s got to be done right if you\u2019re ultimately going to achieve good outcomes.<\/span><\/p>\n<p>Doing hazard ID really well is hard \u2013 it\u2019s easy to focus on the operational phase that\u2019s most familiar, to fixate on one of (or a subset of) the hazards, to identify lots of hazards related to the person\u2019s own work but have no knowledge of the hazards associated with anyone else\u2019s, to assume that personnel will be concentrating on their work as much as the person doing the hazard ID is, to miss things because you\u2019re tired, or to fall prey to any of the cognitive biases that plague the human brain when dealing with complex technical systems.<\/p>\n<ul>\n<li style=\"list-style-type: none;\"><\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][vc_column width=&#8221;1\/2&#8243;][vc_single_image image=&#8221;7566&#8243; img_size=&#8221;full&#8221; css=&#8221;&#8221;][\/vc_column][\/vc_row][vc_row][vc_column][vc_column_text css=&#8221;&#8221;]There are many best practices and techniques that can be applied to help achieve comprehensive hazard identification, though.\u00a0 We don\u2019t have space for an exhaustive list, but a few principles that can be applied when you\u2019re doing hazard ID are:<\/p>\n<ul>\n<li>It can\u2019t be done by one person acting alone \u2013 the H&amp;S manager doesn\u2019t know enough, the machine operator doesn\u2019t know enough, the maintenance engineer doesn\u2019t know enough, and the expensive consultant doesn\u2019t know enough.\u00a0 Together, they should know enough &#8211; like most aspects of achieving good H&amp;S outcomes, hazard ID is a team sport<\/li>\n<li>Use some kind of formal framework to guide hazard ID \u2013 even a simple set of guide words is more robust than just looking at the situation and writing down hazards that come to mind<\/li>\n<li>Ask yourselves \u201cwhat if?\u201d questions \u2013 pose contingency questions, imagine a conceivable action or inaction and then explore what that might lead to.\u00a0 Could harm result? If so, you\u2019ve identified a new hazard<\/li>\n<li>Assume people will make mistakes \u2013 because they will.
